This form can be used to comply with the COVID-19 Tenant Relief Act’s requirement to provide residents who owe unpaid rent that came due between September 1, 2020 – June 30, 2021 with a 15-day notice, provided that the CARES Act eviction moratorium does not apply. Do NOT use this form for charges other than rent. Instead use Form CA-411.
