This form can be used to comply with the COVID-19 Tenant Relief Act of 2020’s requirement to provide residents who owe unpaid charges other than rent that came due between March 1, 2020 – August 31, 2020 with a 15-day notice, provided that the CARES Act eviction moratorium does not apply. Do NOT use this form for unpaid rent. Instead use Form CA-401.
