This form can be used to provide the required annual notice in circumstances where a renewal lease is not entered into annually. This form may also be used to provide notice of service changes.

CAA form SD-001 – Recycling Addendum (City of San Diego) should be provided to all new occupants and to all occupants annually as an addendum to the Rental/Lease Agreement, if it is renewed annually.
